Fees / Insurance

In order to correctly fit these prosthetic devices, a much longer time than a traditional eye examination or contact lens fitting is needed.  Our expert doctors and staff take pride in knowing they always spend the needed time to assure a proper fit and answer all of your questions.  Adjustments and tweaks (and there may be several) of the fit should be expected.

Payment is due when services are rendered.

Scleral Lens Associates, Inc does not accept most  insurance plans. 

We accept Medicare only!  Letters and forms will be provided so you can submit the claim to your insurance company. They should reimburse you for whatever your benefit is.

It is recommended that Patients contact their insurance provider and know what level of reimbursement (if any) BEFORE your appointment so there will be no surprises. Some insurance companies reimburse fully, others not at all and many somewhere in between.

The total cost of a Scleral prosthetic device (per eye) can vary.  The average cost (including device and all professional services) is @ $2000.00 (range @ $1600 –  $2400)/per eye.  The reason for this variation is that some eyes are more challenging than others. Occasionally, due to the nature of Scleral lenses, an unexpected result can occur and it will take more time to resolve it.   Although rare, the cost could be in the $3000 range/eye.

Instead of charging a global fee, each visit and the prosthetic device itself has a separate charge.  This makes it easier for your insurance carrier to understand the fees and hopefully easier for you to be reimbursed.

Initially, a corneal topography will need to be performed.  If you have had this test at your referring doctor’s office, please bring a colored print out.  (A black and white Xerox copy is not acceptable).  Otherwise it will be performed at the initial visit.

One eye is fit at a time during that day’s office visit!  If both eyes need scleral prosthetic devices, the fit will be on different days but they can be dispensed together.  (Exceptions for extenuating circumstances)
